Follower: Talonpriest Ishaal.
To get your follower upon completion of the achievement, head to Veil Terokk. You pick up the quest Talonpriest Ishaal from the NPC of the same name and it will award him as a follower. Obviously, this quest is not available until after you've completed the achievement
