Posted Aug 27, 2016 (Patch 7.0.3)This item unlocks the hidden appearance called Iron Warden for the Vengeance demon hunter artifact,
Aldrachi Warblades. It is looted from any of the mobs summoned via a portal to the Twisting Nether. It requires the Tier 2 Order Hall trait "Twisting Nether." It is not a guaranteed drop and the summoning can only be performed once per day.

Posted Feb 18, 2017 (Patch 7.2.0)In order to get this item you'll first need to have unlocked the Twisting Nether order hall upgrade. From then on you can just grind it daily. But while unconfirmed, I believe that you need to have unlocked all of your class order hall gear before it'll drop.
