Posted Jul 14, 2013 (Patch 5.4.0)The shield only drops in Heroic mode. According to its item page on Battle.net, it has a Medium chance to be looted from Nexus-Prince Shaffar.
The only other item in the game with which this shield shares its model is
Skullflame Shield.
